Claim Form

Please note there is no longer an option to submit your claim form online so you will need to mail your hard copy claim form if you received one. Although the June 4, 2015 deadline for submission of claim forms has passed, the Settlement Administrator may still able to process properly filled out claim forms through July 20th, meaning the claim form must include all required information and be signed properly. Therefore, if you wish to submit a claim form, you are encouraged to do so, but please make sure your claim form is complete and all required documentation is included before it is mailed. However, please note that there is no guarantee that a claim form submitted after the June 4, 2015 postmark deadline will be accepted. To maximize the likelihood that your claim form can be accepted, please mail your claim form as quickly as possible and make sure your claim form is fully complete before it is sent.
